Funding Partners

ETAS endeavors to be as accommodating as possible with the various types of funding sources families may have. Below is a list of funding agencies with which ETAS has/will become affiliated.

Agreement for Services

Prior to the initial visit, a service agreement must be signed by the financially responsible party, parent or guardian.

Payment

For Private/Parent Pay Clients with insurance, ETAS will provide a detailed statement for out-of-network reimbursement. For Private/Parent Pay Clients without 3rd party funding, please contact ETAS at 909.795.4255 for current rates, payment options and administrative discount for pre-payment of services.

To determine whether your child qualifies for funding, please contact your insurance network and inquire about your mental or behavioral health benefits. Some insurance plans may have limitations, including deductibles, co-pays, referrals from the Primary Care Provider or Specialist, maximum therapy sessions per month/year or lifetime expenditure caps.

ETAS accepts MasterCard and Visa credit cards. Payments may also be made by debit card, PayPal or cashier’s check.
